Carers of People with End of life /Life Limiting Illnesses
This service provides emotional, practical and psychological support to carers, family and friends and to those living with cancer and other life-limiting conditions in their own homes. Our objectives are to:
• help avoid unnecessary and unwanted admissions to hospital or hospice for terminally ill patients who wish to remain at home
• support carers to give them a break during the difficult time of caring
We provide support to an individual with personal care needs following an assessment and support plan.
Practical support to carers and their families including shopping, support with nutritional needs and other duties as agreed on an individual basis. We aim to relieve some of the pressure on carers at a very challenging time.
This is an important aspect of our work. Our staff offer a listening ear to the person with care needs and their family and a ‘signposting’ service to other local resources to further support carers to face their caring responsibilities.
Continuity of care
We liaise with other health care professionals to ensure effective continuation of care and have dedicated carer support workers to provide the care at home.
Management is particularly important given the complexity of palliative care. Crossroads Care Wokingham provides services that are delivered in a professional and carer-focused way.
